感觉没有谓语啊。to relate amounts 介词短语做定语修饰code structure。问题是process this claim到底是修饰数据元素还是代码结构，我想是数据元素。然后最后的qualified by...是修饰代码结构还是this claim，不是特别清楚。

我的看法这不应该是一个句子，而是一条短语，其关系为：

“to relate amounts or values to identified data elements”说明“A code structure”：形式为“sth relates A to B”，意为“某事物使A和B相关”

"necessary to process this claim"说明“data elements”，意为“处理这一声明（或要求）所必须的数据元素”

“as qualified by the payer organization”说明“this claim”：意为“由付款机构认可这一要求”

所以所有后面的内容均说明中心成分“A code structure”。
