<p>Informatics Support for the Management of Coronary Heart Disease in Primary Care in Barnsley</p><p>Barnsley PCT Kendray Hospital</p><p>Background:</p><p>Barnsley PCT has developed a set of electronic tools to support the management and care of patients with Coronary Heart Disease (CHD) in Primary Care. The tools facilitate the collection of data during consultations, validate diagnostic data and support the information requirements of the CHD National Service Framework (NSF) and new General Medical Services (nGMS) Contract for General Practitioners. Practices receive an on-going programme of training and support to facilitate use of the tools, encourage improvements in data quality and to help increase understanding of the use of the clinical system to enhance patient experience and care.</p><p>Benefits:</p><p>•  atients:<br />o Receive appropriate treatment<br />o Are not inappropriately called to clinics<br />o Have effective recall and review</p><p>•  ractices<br />o Can work towards improving patient care and review patients who have not attended clinics<br />o Can monitor the care they provide and see improvements in that care<br />o Can produce accurate reporting figures for PCT clinical audit<br />o Can produce accurate reporting figures for NSF and nGMS Contract requirements<br />o Can be sure of appropriate use of resources in running CHD clinics and appropriate prescribing<br />o Can work towards higher targets for quality payments for clinical indicators</p><p>• The PCT<br />o Has accurate information for future planning of resources for CHD management<br />o Has accurate data regarding the incidence and prevalence of CHD<br />o Can accurately address issues of inequalities in care provision</p><p>Transferability:</p><p>The programme has been developed around the functionality of the EMIS clinical system as 38 of 44 practices in the PCT currently use the EMIS system, and 5 are planning to move to EMIS in the near future. The validation and audit tools could be used on any EMIS LV system and will be developed for use with EMIS PCS. The template dataset could be used to design similar data entry forms on any clinical system and the audit and validation tool could potentially be developed for any system that has the function to export search results to MS Excel.</p> |
