care 该如何准确翻译

在加拿大的电子病历蓝图中，care这个词使用的地方很多，从词面上看是照护，护理的意思，但很多情况下看，似乎有不完全是护理的意思，比如下面的：

Additionally, this record is designed
to facilitate the sharing of data – across the continuum of care, across healthcare delivery organizations
and across geographies.

我觉得across the continuum of care，是否翻译为：贯穿持续的医疗过程？
